The ikhankatha is a ritual office within the ulwaluko initiation structure, charged with caring for and overseeing an initiate through the seclusion period following circumcision. Although appointed with the parents' involvement, the caregiver's authority over the initiate during the confinement period at the initiation site is direct and exclusive for that period — the initiate could not leave the site or seek help independent of the ikhankatha's control.

Siyasanga Sifumba, a traditional nurse/caregiver (ikhankatha) appointed by the initiate's parents, was convicted of assault with intent to cause grievous bodily harm and sentenced to two years' direct imprisonment; the court also declared him unfit to possess a firearm.

Reports that the Tsomo Magistrate's Court sentenced Siyasanga Sifumba (30), an ikhankatha (traditional nurse/caregiver) appointed by the initiate's parents, to two years' direct imprisonment for assault with intent to cause grievous bodily harm, after he repeatedly beat a male initiate with a stick and struck him with fists and boots at an initiation site in Sixhotyeni village, Gqogqorha, outside Tsomo, on December 5, 2024. The initiate required hospitalization first at Cofimvaba Hospital, then Frere Hospital, East London. Sifumba was arrested December 17, 2024, denied bail January 23, 2025, and remained in custody through trial.
