Mdludla was both the unregistered traditional surgeon (ingcibi) and the principal of the initiation school — the ritual office that performs and oversees the ulwaluko circumcision rite that inducts Xhosa boys into manhood. Initiates are confined at the school for the duration of the rite and are under the principal's and caregivers' authority for its full course; that authority, not a parent's, is what put the victim inside the school and unable to leave when he began to fail.

Mbizana Regional Court, Eastern Cape, South Africa. Yongama Mdludla, an unregistered traditional surgeon who ran the illegal initiation school as its principal, pleaded guilty to culpable homicide and to contravening the Customary Initiation Act 2 of 2021, and was sentenced to 15 years' imprisonment. Two co-accused who acted as traditional nurses/caregivers at the school, Yamkela Bentswana and Mihlali Khiphi, were sentenced to 8 years each for their roles in the same incident. A separate registered traditional surgeon, Luthando Madikizela, who had circumcised underage boys at the school without required medical certificates, was sentenced to 8 years with 4 suspended for contravening the Act. Press reporting describes the 15-year term as among the heaviest sentences imposed for a traditional-initiation offense in the province since the initiation laws were enacted in 1996.
